Upon registering and paying for a class a welcome packet will be uploaded to your Customer Parent Portal area. There are three forms: General Liability Waiver, Covid-19 Waiver, and Medical Form. Please complete these and upload them to your Parent Portal or mail them to Space Voyage, 1504 S Johnson Ct, Lakewood, CO 80232. Every student is required to have these forms on file to attend. Thank you. The form below is for online or virtual programs. To see the three summer camp forms, click on home at top pf page, then the FORMS tab on the top menu. You can use the pull-down tab to view the forms.
